2024-10-22 Resolution 12261RESOLUTION NO. 12261 RESOLUTION DESIGNATING CITY'S CONTRIBUTION TOWARD HEALTH, DENTAL, TERM LIFE, AND DISABILITY INSURANCE PREMIUM FOR GENERAL SERVICES AND MANAGEMENT EMPLOYEES WHEREAS, the hospital-medical/surgical group health insurance plan is available from the LOGIS Health Insurance program for City employees and their dependents; and WHEREAS, a self-funded group dental insurance plan is available to full-time City General Services and Management employees and their dependents; and WHEREAS, a term life and accidental death and dismemberment insurance plan is available from the Local Government Information Systems Association (LOGIS) for full-time City General Services and Management employees and their dependents; and WHEREAS, a group long-term disability program is available to full-time City General Services and Management employees; and WHEREAS, the City Council is required to determine by resolution the City's contribution toward the premium for employee group insurance coverages and approve agreements other services offered to employees. NOW, THEREFORE, BE IT RESOLVED that for those employees working 30 or more hours weekly, the City shall contribute between $880.00 to $1,059.00 per month for individual health insurance, as well as an HSA contribution with a maximum of $200 per month The City will contribute between $1,839.84 to $1,922.84 per month for family and $1,572.63 to $1,642.38 for dependent coverage. For part-time employees working 20-29 hours weekly, the City shall contribute between $782.13 to $914.00 per month towards individual health insurance, $910.82 to $993.82 per month towards family coverage and $815.46 to $885.21 for dependent coverage. Actual City contribution will be dependent on the plan selected by the employee. For all full-time General Services and Management and all bargaining unit employees, the City shall also pay the $60.25 monthly premium for the offered dental insurance plan, and the $1.54 monthly premium for the term life and accidental death and dismemberment insurance plan. Such contributions shall be for coverage effective January 1, 2025. BE IT FURTHER RESOLVED that in 2023, the City entered into an agreement with Madison National Life/ Ochs Company for a two-year period at a cost of $0.18 per $100 of covered payroll. The City shall continue to contribute the full cost of long-term disability insurance for all full-time employees. BE IT FURTHER RESOLVED that the City Council shall determine the City's contribution toward insurance premiums for all organized employee groups by the adoption of the appropriate resolutions concerning the labor contract with the respective organized employee groups.
