RESOLUTION DESIGNATING CITY'S CONTRIBUTION TOWARD HEALTH, DENTAL, TERM LIFE,
DISABILITY INSURANCE and the EMPLOYEE ASSISTANCE PROGRAM
PREMIUM FOR ALL BENEFIT ELIGIBLE EMPLOYEE GROUPS
WHEREAS the hospital-medical/surgical group health insurance plan is available from the LOGIS
Health Insurance Program for City employees and their dependents; and
WHEREAS a self-funded group dental insurance plan is available to full-time benefit eligible
employees and their dependents; and
WHEREAS a term life and accidental death and dismemberment insurance plan is available from
the Local Government Information Systems Association (LOGIS) for full-time benefit eligible employees
and their dependents; and
WHEREAS a group long-term disability program is available to full-time benefit eligible
employees; and
WHEREAS the Employee Assistance Program (EAP), provided through Sand Creek, is available to
all full-time and part-time employees and will be expanded to include intermittent employees effective
January 1, 2026; and
WHEREAS the City Council is required to determine by resolution the City's contribution toward
the premium for employee group insurance coverage and approve agreements other services offered to
employees.
NOW, THEREFORE, BE IT RESOLVED that Attachment A outlines the premium costs for each
plan, the City’s contribution, and the employee’s share of the cost. The rate changes will apply to all
benefit-eligible employees working 30 hours or more per week, as well as to part-time employees
working between 20 and 29 hours per week. In addition to the City’s contribution toward premiums, the
City also provides Health Savings Account (HSA) contributions for eligible plans, up to a maximum of
$200 per month. Actual City contribution will be dependent on the plan selected by the employee.
For all full-time employees, the City shall also pay the $60.25 monthly premium for the dental insurance
plan offered. Such contributions shall be for coverage effective January 1, 2026.
BE IT FURTHER RESOLVED that in 2026, the City entered a two-year agreement with MetLife
for long-term disability insurance coverage for at a cost of $0.144 per $100 of covered payroll. The City
shall continue to contribute the full cost of long-term disability insurance for all full-time employees.
In addition, for 2026, the City entered into an agreement with LOGIS for a 3-year contract with
Securian Financial for Life/ADD at $4.73 per month per employee. The City shall contribute to the full cost
of the Life/ADD. The 2025 premium rates were reduced for both LTD and Life/ADD effective 2026.

BE IT FURTHER RESOLVED that the City Council has determined the City's contribution toward
insurance premiums for all organized employee groups are included in the benefit eligible employees.
