H0SPI1'AL~MEDICAL INSDRANCE PREMIUM FOR IN`TERNA`TIONAL ASSOCI~
AI'ION OF FIREFIG'H'TERS LOCAL 1215
WHEREAS, a hospita,l~medical/surgical group insurance plan is available from
the Hennepin County Cooperative Purchasing Insurance Group for city employees and
their families, and
WHEREAS, all permanent full®time employees and permanent probationary employees
shall be eligible for plan benefits as described in the Personnel Rules a.nd Regu~
lations Ordinance, and
WN.ER~AS, the City Council is required by resolution to determine the city`s
contribution toward a single person premium, and
WHEREAS, Resolution No. 4892 designated the city`s contribution toward a single
person premium for all city employees except members of Firefighters Local 1215,
NOW, 'TN~REFORE BE I1' RESOLVED that the City Council of the City of Richfield
authorize the Firefighters Local 1215 to participate in the Hennepin County Cooper-
ative Purchasing Insurance Group,
BE I~ FUR`THER RESOLVED that the city shall contribute $19.04 per month toward a
single person premium for all eligible employees, ($228.48 per annum), with the
participating employee paying any remaining premium charge through payroll deductions.
