1975-02Bi11 1975-2 AMENDMENT TO SECTION 2.35 OF THE ORDINANCE CODE OF THE CITY OF RICHFIELD CITY OF RICHFIELD DOES ORDAIN: Section 2.35 of the Ordinance Code of the City of Richfield, Minnesota, relating to group hospitalization insurance is hereby amended to read as follows; "2.35 GROUP INSURANCE PROVISIONS Subdivision 1. Hospital - Medical/Surgical Insurance Group Plan, Such an insurance plan is available for eligible city employees and their families. (1) Eligibility for Insurance Plan Benefits, A11 permanent full-time employees and permanent probationary employees shall be eligible for insurance plan benefits of the city's Hospital-Medical( Surgical Insurance Group Plan, (2) Premium Payments, The total monthly premium for participating employees shall be paid in the following manner: (a) The city council shall by resolution determine_th.e city's /annual/ contribution towards /a single person/ the premium. (b) The participating employee shall pay any remaining premium charge through payroll deductions. (3) Effective Date of Coverage. New eligible employees who elect to participate shall be covered on the first of the month following their date of employment, (4) Discontinuation of Coverage, Whenever a covered employee ceases employment with the city, insurance coverage will be discontinued on the first of the month following the date of termination. (5) Coverage on Leave of Absence Without Pay. An employee can elect to continue coverage while on leave of absence without pay. (8) Said newspaper has filed with the Secretary of State of Minnesota prior to January 1, 1966 and each January 1 thereafter an affidavit in the form prescribed by the Secretary of State and signed by the publisher of said newspaper and sworn to before a notary public stating that the newspaper is a legal newspaper.
