RESOLUTION DESIGNATING CITY'S CONTRIBUTION TOWARD
HEALTH, DENTAL, TERM LIFE, AND DISABILITY INSURANCE
PREMIUM FOR GENERAL SERVICES AND MANAGEMENT
EMPLOYEES
WHEREAS, the hospital-medical/surgical group health insurance plan is available from the
LOGIS Health Insurance program for City employees and their dependents; and
WHEREAS, a self-funded group dental insurance plan is available to full-time City General
Services and Management employees and their dependents; and
WHEREAS, a term life and accidental death and dismemberment insurance plan is
available from the Local Government Information Systems Association (LOGIS) for full-time City
General Services and Management employees and their dependents; and
WHEREAS, a group long-term disability program is available to full-time City General
Services and Management employees; and
WHEREAS, the City Council is required to determine by resolution the City's contribution
toward the premium for employee group insurance coverages and approve agreements other
services offered to employees.
NOW, THEREFORE, BE IT RESOLVED that the City shall contribute a maximum of $1,605
per month for family health insurance to full-time employees, and in any event, said contributions
shall not exceed the cost of single coverage for employees selecting that option. For all full-time
General Services and Management bargaining unit employees, the City shall also pay the $60.25
monthly premium for the offered dental insurance plan, and the $5.95 monthly premium for the
term life and accidental death and dismemberment insurance plan, for a total possible maximum
2022 insurance premium contribution of $1,671.20 per month. For part-time regular
employees the City shall contribute 75% of the single health care city contribution provided to
full-time employees and a maximum of $803 per month towards dependent coverage. Such
contributions shall be for coverage effective January 1, 2023.
BE IT FURTHER RESOLVED that in 2021, the City entered into an agreement with
Madison National Life/ Ochs Company for a three-year period at a cost of $0.18 per $100 of
covered payroll for an annual premium of $29,302.00. The City shall continue to contribute the full
cost of long-term disability insurance for full-time General Services and Management employees.
BE IT FURTHER RESOLVED that the City Council shall determine the City's
contribution toward insurance premiums for all organized employee groups by the adoption
of the appropriate resolutions concerning the labor contract with the respective organized
employee groups.
