RESOLUTION NO.
RESOLUTION DESIGNATINGCITY'S CONTRIBUTION
TOWARD HEALTH, DENTAL,TERM LIFE, AND DISABILITY INSURANCE
PREMIUM FOR GENERAL SERVICES AND MANAGEMENT EMPLOYEES
WHEREAS, thehospital-medical/surgical grouphealthinsuranceplanisavailablefrom the
LOGIS HealthInsuranceprogram forCity employeesand their dependents; and
WHEREAS,aself-funded groupdental insuranceplanisavailabletofull-timeCity General
ServicesandManagement employeesandtheir dependents; and
WHEREAS,aterm lifeandaccidental deathand dismemberment insuranceplanisavailable
from theLocal Government InformationSystemsAssociation(LOGIS) forfull-timeCity General
ServicesandManagement employeesandtheir dependents; and
WHEREAS,agrouplong-term disability program isavailabletofull-timeCity General Services
andManagement employees; and
WHEREAS, theCity Council isrequiredtodetermineby resolutiontheCity's contributiontoward
thepremium foremployee groupinsurancecoveragesand approveagreementsother servicesoffered
toemployees.
NOW, THEREFORE, BE IT RESOLVEDthat theCity shall contributeamaximum of $1,500per
monthforfamily healthinsurancetofull-timeemployees, andinany event, saidcontributionsshall not
exceedthecost of singlecoverageforemployeesselectingthat option. For all full-timeGeneral
ServicesandManagement bargainingunit employees, theCity shall alsopay the$60.25 monthly
premium fortheoffereddental insuranceplan, andthe$1.54 monthly premium fortheterm lifeand
accidental deathand dismemberment insuranceplan, foratotal possiblemaximum 2022 insurance
premium contributionof $1,561.79 permonth. TheCity shall contribute75%of thesinglehealthcare
premium forpart-timeregularemployeesand a maximum of $754 permonthtowardsdependent
coverage. Suchcontributionsshall beforcoverageeffectiveJanuary 1, 2022.
BE ITFURTHER RESOLVEDthat in2021, theCity entered intoanagreement withMadison
National Life/ OchsCompany forathree-yearperiodatacost of $0.18per$100of coveredpayroll for
anannual premium of $27, 310.95. TheCity shall continuetocontributethefull cost of long-term
disability insuranceforfull-timeGeneral ServicesandManagement employees.
BE IT FURTHER RESOLVED that the City Council shall determine the City's contribution
toward insurance premiums for all organized employee groups by the adoption of the appropriate
resolutions concerning the labor contract with the respective organized employee groups.
